Please note:

In line with government legislation, any holiday requests received will not be authorised (except in exceptional circumstances).

The holiday request absence for must be completed 3 weeks before the first day of the requested holiday. All holidays (regardless of whether it is likely they will unauthorised or not) must be registered with the school. In the event of the Local Authority issuing you with a fine, the information that you submitted (detailing the reasons for taking holiday during term time) could be used to support your appeal against the fine.

    Please include the initials of the children and the class that the holiday request is for...
    Please include details of children who may be attending other schools.

    Note: This is because we may need liaise with the other school to find out whether they have authorised the absence or not. We will need to physically speak with the Secondary School, as in some cases parents tell us the other school have authorised the absence when they haven't.
    I request that you consider allowing my child (ren) to be absent from school for the following dates:

    Note: Any holidays of three days or less, which are not authorised, will not lead to a fine.



    Note: If you are issued a fine by the local authority the information you include in this box could be used to support your appeal.
    Note: This information is required for the absence request. In the event of children, whose parents are separated, both parents will need consent to the absence. This prevents one parent from taking a child on holiday without the other parent being aware or consenting for the child to be away from school.
